Home and Community Based Services

Attendant Care: Provides a trained direct care worker (DCW) to assist a member to create or maintain safe and healthy living conditions. This assistance may include maintaining personal cleanliness and activities of daily living.  They provide essential services that ensure that your loved one has their needs met in the home and are also able to participate in engaging in activities within the community.   Attendant Care services are personalized for each client based on their individual needs for support.

Habilitation: Provides methods and training to increase a member’s ability to become more independent and improve upon life skills. The goal is to assist your loved one develop skills based on their individual needs that have included family/team input.  The focus can include, but is not limited to: self-help, socialization, and adaptive skills to live and participate in the community.   Habilitation may be provided in a member’s home or in their community.

Respite Care: Provides a trained person to supervise and care for a member so that their caregivers can have time for self care and to get relief.  Respite can also be used as a fun opportunity to take the individual out in the community to enjoy leisure recreation activities like going to the park, sports activities, movies, or other activities the client might enjoy.

Homemaking: Provides support for a variety of housekeeping tasks. It allows a member to have their home kept in a clean and healthy condition.
